$40, cash, sports physicals are NOT billable to insurance

Physicals include a sports medicine screening by a board certified doctor and licensed athletic trainer. They do not include blood tests or immunizations. One physical per appointment for students up to age 18. Please complete the health history portion of the physical form before the appointment. Participants should come dressed in shorts and bring their glasses or contacts. Pre-registration required; no walk-ins allowed.
